
The amount of divorce paperwork required as part of a divorce can be quite overwhelming. Here is a list of the forms needed to go from divorce petition to Decree Absolute. If you would like to know the divorce process or how to divorce, then contact us.
D8 Form – Divorce Application
This is the initial form required to start the process. The petitioner will complete this form and send it to court. This form is no longer required if you are using the new Government Online system. Send 3 copies of the form to your nearest divorce centre. They will send one copy to your husband/wife.
You need to send 4 copies if you named someone your husband or wife committed adultery with.
Keep your own copy of the forms.
Form D10 – Acknowledgement of Service
This form will be sent to your husband or wife along with a copy of the D8 Form. They (the respondent) has 7 days to respond to the form to notify the court whether they are going to defend the application. If an adulterer has been named, then they will also receive a copy of the Acknowledgement of Service.
D8B Form – Answer a Divorce Application
This is the form required to answer a D8 form if the respondent intends to defend the application for divorce.
D11 Form – Apply to the court for an interim order within your divorce proceedings
This is the form required to answer a D8 form if the respondent intends to defend the application for divorce.
Form D13B
Ask the court to dispense with service in divorce or legal separation proceedings
Use this form to request the court to proceed with your divorce proceedings even though the respondent could not be served with the papers. Only use this form as a last resort and you have exhausted every possible avenue in trying to find your spouse.
Form D84 – Apply to the court for a decree nisi, conditional order or (judicial) separation decree or order
A decree nisi or conditional order is the first of two decrees or orders you must have before you are finally divorced or your civil partnership is finally dissolved. A judicial separation decree or separation order is the only and final document issued by the court.
You can use this form to apply whether or not your partner agrees to your petition. There is no fee payable for this application.

Form D80 Support Statements
One of the following is required to be submitted with the Form D84. Select and complete one of the following D80 forms
Form D80-A
Statement in support of an application for divorce on the ground of adultery.
Use this form if you intend to use your spouse’s adultery to support your application to legally end your marriage or civil partnership.
Form D80-B
Statement in support of an application for divorce on the ground of unreasonable behaviour.
Use this form if you intend to use your spouse or civil partner’s unreasonable behaviour to support your application to separate or end your marriage or civil partnership.
Form D80-C
Use this form if you intend to use your spouse’s desertion to support your application to legally end your marriage or civil partnership.
Form D80-D
Statement in support of an application for divorce on the ground of two years’ separation
Use this form if you have been separated for at least 2 years and that you both consent to ending the relationship.
Form D80-E
Statement in support of an application for a divorce on the ground of 5 years’ separation
Use this form if you been separated for at least 5 years as the ground to legally end your marriage or civil partnership. This does not require consent of your spouse.
Form D36
Provide information to make a decree nisi absolute, or a conditional order final
Use this form to apply for a final order to legally end your marriage or civil partnership.
Form D89
Use this form to ask a court bailiff to hand-deliver papers to the respondent in family proceedings for you.
